Access to the bunker doors leading to the hospital is strictly restricted, limited to Level 4 personnel or higher. The doors are to remain securely locked at all times. In the event of a lockdown, personnel must promptly evacuate the immediate vicinity, and surveillance cameras will be utilized to monitor the movements of DA-116-A until the lockdown ceases.

DA-116 constitutes an anomalous containment facility located beneath the nuclear bunker of Ex-Site-██. Accessible through the bunker doors, the shelter extends into an abandoned hospital section known as Ward B, surpassing the dimensions of the hospital's surface structure. Despite multiple explorations, the true scope of DA-116's interior remains elusive.
DA-116-A, the primary anomaly within the containment facility, manifests during lockdown events within Ward B. Strikingly, it materializes despite the absence of any apparent means of entry. This entity presents as a tall, slouched humanoid, measuring approximately 2.5 meters in height. Its exterior, a dark brown hue, seamlessly blends with the shadows of the hospital, while its eyes emit a luminescent white glow, aiding in navigation through poorly lit environments.
Exhibiting predatory behavior during lockdown events, DA-116-A actively seeks individuals within Ward B. Navigating the labyrinthine halls with purpose, it utilizes heightened senses to detect potential hiding spots. Victims encountering DA-116-A during lockdowns report intense feelings of dread and paranoia. Some individuals have died due to heart attacks due to these events. Notably, the entity vanishes upon the cessation of lockdown, leaving no trace of its presence.
Addendum:
The discovery of DA-116 transpired following reports of peculiar incidents near ████████. Local authorities, suspecting trespassers or criminal activity, initiated an investigation into the abandoned hospital. Foundation operatives, integrated within local law enforcement, intercepted these reports and initiated containment procedures. Subsequent exploration uncovered the entrance to DA-116 within the nuclear shelter.
Initial investigations disclosed the anomalous properties of Ward B and the manifestation of DA-116-A during lockdown events. Ongoing containment efforts center on monitoring and securing the anomaly to prevent civilian exposure.
Incident DA-116-01
On ██/██/20██, DA-116-A was found to be roaming outside of DA-116, and instead was roaming in Ex-Site-██. This behaviour was not particularly noticed before, therefore, no containment procedures were put in place.
The CCTV cameras outside of the bunker failed to work that day, meaning motion sensors captured the movements of DA-116-A.
DA-116-A then proceded to wander outside of the facility, which was caught by CCTV.
DA-116-A was then caught and re-contained in “Ward B”. More safety precautions have been put in place to stop this from happening in the future.
